Biography
Professor Bas Wijnhoven is a upper GI surgeon working at the Erasmus University Medical Center Rotterdam (the Netherlands). He is program leader on oesophageal and gastric diseases. Research is focused on outcomes after surgery for gastric and oesophageal cancer and clinical studies on multimodal treatment for oesophageal and gastric cancer. Professor Wijnhoven is currently the chair of the guidelines committee of the ISDE, chair of the Dutch Upper GI Cancer Group and editor for BJS (British Journal of Surgery; IF 8.6). He has authored and co-authored over 350 articles in national and international peer-reviewed journals
Summary of presentation
GEJ cancer is a challenging disease from the viewpoint of the surgeon. Given its location ate the junction between the esophagus and stomach, surgical treatment can be either (extended) total gastrectomy or esophagectomy. The decision to opt for one or the other is guided by several considerations: can de tumor and lymph nodes be resected with a tumor-free resection margin (extent of invasion in esophagus/stomach and nodal stations involved)? What type of reconstruction can be safely performed? What surgical apprach is associated with the lowest morbidity/mortality? Do I, as a surgeon, feel competent to do this operation and am I able to adopt my surgical strategy when intra-operative findings require this (proficient in the abdomen, chest and neck)? To guide the surgeon, optimal pretreatment staging with endoscopy, sometimes EUS and imaging can be of importance. Diagnostic laparoscopy may be of value in selected patients. In my presentation I will discuss thee consideraions and give an overview of the current literature.
